The patchnotes from 1.1.2 suggest some corrections to the item, so it was definitely there before. As I've never found it myself (with some odd 170 total hours in Dredmor), I can surmise something weird was preventing it from appearing. It is almost certainly from CotW, though.
It was added with the patch. The graphic was indeed finished for the release of CotW but finishing that particular item xml slipped through the cracks. There are a couple more new items in there to find
